Re-instate a license (license expired less than 2 years)
You can renew a license for up to 24 months from the expiration date through the DFS Portal.  After 2 years, you will need to re-test.

Duplicate license request form (electronic)
Duplicate licenses are available to current New York DFS licensees who have lost or misplaced their insurance license.  Duplicate licenses are available through the DFS Portal.  Once logged in, click the printer icon to the far right of your license information.  You will be prompted to affirm you request and a fee of $15 may be applied.

Change of Address form
Address changes are processed on the DFS Portal page.  Login with your license number and password and select the Address Change option.  Please note that address changes can be made anytime, not only at renewal.
